Baxter recalls four batches of hospital IV fluids in a month over contamination
The maker of most of the nation's intravenous fluids has pulled saline, dextrose and a blood-thinning solution after finding fiberglass, steel and cardboard fragments inside sterile bags, reviving concerns about a supplier hospitals can't easily do without.
Baxter International, the company that supplies roughly six in ten of the intravenous fluid bags used in American hospitals each day, has recalled four separate lots of injectable solutions since late July after workers found stray fragments of glass fiber, stainless steel and cardboard inside bags meant to be sterile and free of any foreign material. The three most recent recalls were announced within a single 24-hour span this week, covering saline, a concentrated sugar solution and a blood-thinning fluid used in blood-donation machines, and reached hospitals and clinics in at least 17 states.
No injuries or deaths have been tied to any of the affected lots, according to the notices Baxter filed with the Food and Drug Administration. But the clustering of four particulate-contamination recalls in roughly five weeks has drawn notice from hospital pharmacists still wary after a Baxter manufacturing disruption two years ago touched off a nationwide IV shortage.
What was recalled
The newest recall, made public Wednesday, covers two lots of Baxter's 0.9% sodium chloride injection — ordinary saline, one of the most widely used products in any hospital — in 500-milliliter bags, after the company detected fiberglass particles in the solution. The lots, numbered Y495523 and Y495523A, were distributed between July 31 and Aug. 3 to health systems and distributors in Florida, Illinois, Indiana, Louisiana, Maryland, Missouri, Nevada, New Jersey, New York, North Carolina, Ohio, South Carolina, Texas and Virginia.
A day earlier, Baxter recalled one lot of its 70% dextrose injection, a concentrated sugar solution used in nutrition support and to treat severe low blood sugar, after finding stainless steel particles in a 2,000-milliliter bag. That lot, Y495066, was shipped to providers in Alabama, Georgia and Tennessee between July 25 and 28.
The same day, Baxter recalled a lot of its anticoagulant sodium citrate solution, which is used with automated apheresis machines that separate components of donated blood, again citing fiberglass contamination. That lot had gone to a single customer in the United States in early July.
These three followed a recall in late July, when Baxter withdrew a lot of cefazolin in dextrose injection, an antibiotic-and-fluid combination bag, after finding a piece of cardboard inside — bringing the total to four contamination recalls of injectable Baxter products in about a month.
Each recall notice describes the same underlying hazard. Material that would never otherwise enter a person's bloodstream can act like debris carried through the veins, where it may lodge in small blood vessels, trigger inflammation or set off clotting.
"Intravenous administration of 70% Dextrose Injection containing stainless steel particulate matter may lead to serious adverse health consequences. Particulate matter can cause blockage and clotting in blood vessels which can cut off blood flow to vital organs, lead to a life-threatening blood clot in the lungs (pulmonary embolism) and cause permanent organ damage or death," Baxter said in the recall notice posted by the FDA.
The company has told the FDA that none of the four recalls has produced a confirmed adverse event so far, and that each contamination was caught by Baxter's own internal testing before the affected lots were traced back through the distribution chain — not through a report of patient harm.
Who is affected
The recalled lots are a small fraction of Baxter's overall output, and the company says it is contacting affected customers directly and arranging for the return of unused product. But the reach is still substantial: combined, the three most recent recalls touched distributors and hospitals across more than a dozen states in the South, Midwest, Mid-Atlantic and Northeast, plus at least one blood-donation or apheresis center whose location has not been publicly disclosed.
Ordinary saline and dextrose solutions are used on general medical and surgical floors, in emergency departments and during routine procedures, meaning the population exposed to any given lot is not a narrow one — it is whichever patients happened to receive fluid from those specific bags in the days before the contamination was discovered. Baxter and the FDA say hospitals should check their own inventories against the recalled lot numbers before administering any of the affected products.
The American Hospital Association flagged the July cardboard-contamination recall to its member hospitals as it was announced, a step trade groups typically take to make sure supply-chain and pharmacy staff see recall notices quickly rather than waiting for them to surface through routine FDA channels.
The shadow of the last shortage
The recalls land against a backdrop of lingering caution about Baxter's supply chain. In September 2024, flooding from Hurricane Helene shut down the company's plant in North Cove, North Carolina — the single facility that produces roughly 60% of the IV solutions administered daily in the United States — and set off a months-long nationwide shortage that forced hospitals to ration saline bags, switch to oral hydration where possible and delay some elective procedures. That plant has since resumed production, and none of the newly recalled lots has been linked publicly to a specific manufacturing site, but the episode left hospital supply officers with little tolerance for repeated disruptions from a supplier so many of them depend on for a single, unglamorous but essential product.
Baxter has not issued a public statement beyond the individual recall notices themselves, each of which lists a media contact for further inquiries and directs affected customers to the company's healthcare service center for instructions on returning product.
What happens next
The FDA has not yet published a formal risk classification — Class I, II or III, in ascending order of urgency — for the three most recent recalls; that determination typically follows in the days or weeks after a company initiates a voluntary recall, once the agency has assessed the likelihood and severity of harm. Hospitals that received the affected lots are expected to quarantine and return the product under their own recall procedures rather than wait for that classification.
For now, the practical guidance from Baxter and the FDA is unchanged from any drug recall: check lot numbers against inventory, stop using anything on the list, and report suspected reactions to the FDA's MedWatch program. Whether the four incidents in five weeks reflect a specific, fixable lapse at one production line or a broader strain on Baxter's manufacturing quality controls is a question hospital pharmacy groups and, eventually, FDA inspectors are likely to keep asking.
