ADHD Linked to Sharply Higher Rates of Gut Problems, Largest Review Finds

A pooled analysis of nearly 2 million people found constipation, irritable bowel syndrome and loss of bowel control all more common among people with ADHD, prompting researchers to urge doctors to start asking.

A systematic review of nearly two million people has found that attention-deficit hyperactivity disorder is associated with substantially higher rates of gastrointestinal problems, from constipation and irritable bowel syndrome to loss of bowel control, in a pattern researchers say has been overlooked in a condition usually discussed in terms of behavior and attention rather than digestion.

The review, led by researchers at Warwick Medical School and published in the Journal of Attention Disorders, pooled data from 23 separate studies covering more than 1.9 million people across the United States, Sweden, Germany and South Korea, ranging in age from 2 to 65. It is described by its authors as the first systematic review devoted specifically to gut symptoms in people with ADHD, rather than a secondary finding folded into broader research on the disorder.

The numbers

Across the pooled studies, people with ADHD were found to have roughly 50 percent higher odds of gastrointestinal symptoms overall compared with people without the diagnosis, according to the University of Warwick. The associations were sharper for specific conditions: nearly double the odds of constipation, a 58 percent increase in irritable bowel syndrome, and more than four times the odds of encopresis, the medical term for loss of bowel control. Indigestion and other functional digestive complaints also turned up more often in the ADHD groups than in comparison groups.

The authors are careful to describe these as associations rather than proof of cause and effect. The review cannot say whether ADHD itself produces gut symptoms, whether medications used to treat it are responsible, or whether some third factor, such as diet or anxiety, drives both conditions at once. Untangling those possibilities, the researchers say, will require further study designed specifically to separate the biology of ADHD from the side effects of its treatment.

How the two conditions may be connected

ADHD affects an estimated 11.7 percent of children ages 3 to 17 in the United States, or about 7 million children, according to data compiled by the Centers for Disease Control and Prevention, with diagnoses in adults numbering in the millions more. The Warwick team's interest in gut health grew out of a body of research on the so-called gut-brain axis, the network of nerve, hormonal and microbial signaling that connects the digestive system to the brain, most prominently through the vagus nerve.

Researchers pointed to several plausible mechanisms rather than a single cause. Stimulant medications commonly prescribed for ADHD, including methylphenidate and amphetamine-based drugs, are already known to list constipation and appetite changes among their side effects, which could account for at least part of the pattern. Impulsivity and irregular eating habits associated with ADHD may also disrupt digestion independent of any medication. Separately, some studies have found differences in the composition of gut bacteria between people with and without ADHD, raising the possibility that the microbiome itself plays a role in the relationship, though the review stops short of endorsing any one explanation over the others.

"Almost every gut symptom we looked at, constipation, IBS, even indigestion, appeared to be associated with ADHD. These symptoms can have a real impact on daily life, but could easily be overlooked," said Sarah Blake, a medical student at Warwick Medical School and co-lead author of the review.

Who is affected

Because the pooled data spanned childhood through late adulthood, the findings apply broadly rather than to one narrow age group. Children with ADHD who also struggle with chronic constipation or stomach pain may have those complaints attributed to picky eating or anxiety rather than connected to their diagnosis, the researchers suggested, while adults with ADHD navigating irritable bowel syndrome may not think to mention it to the clinician managing their attention or focus medication. Encopresis, though less common than constipation or IBS, carries particular social and emotional weight for the children and families who experience it, and the more than fourfold increase in odds found in the review was among the largest effects reported.

The review's geographic spread, drawing on cohorts from the United States, Scandinavia, continental Europe and East Asia, suggests the association is not confined to one health system or diagnostic culture, though the authors note that most of the underlying data are observational and vary in how rigorously gastrointestinal symptoms were measured from one study to the next.

Reaction

Dr. Saran Shantikumar, an associate clinical professor at Warwick Medical School and the review's senior author, said the strength and consistency of the pattern across so many studies was itself notable, even without a confirmed mechanism.

"We can't yet say ADHD causes these gut problems, or exactly what is driving the link, but the pattern is striking enough to warrant attention. Clinicians caring for people with ADHD may want to consider asking about gastrointestinal symptoms, particularly if they may be affecting quality of life," Shantikumar said.

Coverage of the review by outlets including ScienceDaily and News-Medical echoed the authors' emphasis on clinical awareness over alarm, framing the results as a prompt for doctors to ask a question rather than evidence of a new complication that patients should fear.

What happens next

The Warwick team has called for prospective studies that track people with newly diagnosed ADHD over time, ideally comparing those on medication with those who are not, to help separate the contribution of drug side effects from any underlying biological link. Research into the gut microbiome of people with ADHD is also continuing separately, part of a broader wave of studies over the past decade examining how gut bacteria may influence conditions ranging from depression to autism spectrum disorder.

In the meantime, the practical recommendation from the study's authors is modest: that physicians treating patients for ADHD routinely ask about constipation, bowel habits and stomach discomfort, and that patients experiencing persistent digestive symptoms be referred to a dietitian or gastroenterologist rather than assuming the symptoms are unrelated to their diagnosis. Whether that connection turns out to be driven by medication, behavior, biology or some combination of the three remains, by the authors' own account, an open question for the next round of research.
